3.  2 Valid Government and Company ID’s – with 3 specimen signature
4.  Birth Certificate / Marriage Contract (Photocopy)
5.  Residence Certificate (Cedula)
6.  ITR 2316 (except for OFW)
7.  Certificate of Employment w/Compensation or Job Contract  - duly notarized
8.  Proof of Billing (original)
9.  36 Post-dated Checks for Equity (12 pcs. Blank info for UCPB)

HDMF OR PAG-IBIG FINANCING:
1.  Approved Membership Verification Slip (MSVS)
2.  5 pcs. 1x1 ID pictures with white background
3.  Signed Loan Documents
4.  2 mos. Latest Payslip/ Proof of Income
5.  2 Valid Government and Company ID’s – with 3 specimen signature
6.  Birth Certificate/ Marriage Contract (Photocopy)
7.  Residence Certificate (Cedula)
8.  ITR 2316 (except for OFW)
9.  Certificate of Employment w/ Compensatio or Job Contract duly notarized
10.  Proof of Billing (Original)
11. SPA (for Non-Principal Borrowers – Consularized for OFW)
12.  36 pcs Post dated Checks (12 pcs. Blank info for HDMF)

IN-HOUSE FINANCING
1.  2 months latest Payslip or Proof of Income
2.  2 Valid Government and Company ID’s-w/ 3 specimen signature

For Self-Employed:
     1. Financial Statement and Audited Report by the CPA
     2.  Business Permit
     3.  DTI or SEC
For OFW:
      1.  Latest Job Contract / Employment Contract
           For expired job contract – Latest Certification of Job Contract
      2.  SPA w/Red Ribbon/ Stamped by Consulate
